Donating securities, including stocks, bonds, and mutual funds is an efficient and easy way to support SickKids today. Simply fill out the online form below, including your broker’s contact information so that they will receive a copy of the form and work with you to initiate the transaction.
Please note, filling out this form does not initiate the donation. This form is required to ensure your gift can be identified, processed and receipted.
Submitting this form does not initiate or complete your gift of securities. After submitting the form, you must contact your broker or investment advisor to initiate the transfer, or if you have a self-directed investment account, complete the transfer through your investing platform.
3 Simple Steps.
Step 1: Complete the Direction and Notification Form for Gifts of Securities.
Fill all required sections of the transfer form and submit the form.
Submitting this form is mandatory and notifies SickKids Foundation of your intended gift but does not initiate the transfer of your securities.
Step 2: Initiate the transfer through your broker or self-directed investment account
After you submit the form, you and your broker or investment advisor will receive an email with a copy of your completed form and instructions for the next steps.
Your broker or investment advisor must then initiate the transfer of the securities to The Hospital for Sick Children Foundation.
If you are a self-directed investor, please complete the transfer through your investing platform.
Step 3: SickKids Foundation and the broker complete the transfer and issue the donation receipt.
SickKids Foundation’s broker will work with your broker and/or investment advisor to complete the transfer process. Please be advised that the value of the donation receipt will be based on the closing price of the security, on the day the securities are received by the Foundation’s broker.
Once proceeds are received, we will issue your charitable tax receipt, if applicable.
Please note that SickKids Foundation is unable to issue a charitable tax receipt until we are aware of your donation. In order to ensure prompt and accurate receipting of your donation, prior to transferring securities to our account, please complete and submit the form above. Please be advised that the value of the donation will be based on the closing price of the security, on the day the securities are received by the Foundation’s broker, in accordance with our Board Policy and Revenue Canada Regulations.
